Overview of position

Treatment of severe acute malnutrition (SAM) is one of the key components of PND activities serviced through BPHS NGOS. It is primarily implemented in the emergency affected provinces where the prevalence of SAM is high. The services will be expanded as part of emergency response once there is emergency induced SAM cases. Therapeutic nutrition supplies (e.g F-75, F-100 and RUTF) are core elements of the services. PND manage the overall supply chain including forecasting, requesting, distributing, and collecting the reports on a quarterly basis.

Purpose of the post:

The main objective of this assignment is to support PND to manage quality IMAM services delivery as inpatient care and outpatient care. Also, to manage the therapeutic supply and minimize the nutrition supply stock outs through improved nutrition supply management system. The technical assistance will also be responsible for emergency related activities in PND including coordinating with Nutrition cluster. 

Job Description

In September 2021, an IASC humanitarian system wide emergency activation of L3 response (to an L3 emergency) was announced in Afghanistan following transition of power in the country. Our clients kealth program worked with their partners to continue provision of essential health services for children & women in need, across the country. Services are being provided through health facilities, outreach from health facilities & mobile teams for remote & hard to reach areas. Supportive supervision, monitoring are key functions to verify program implementation, improve quality of service, including primary & secondary health care, Maternal, Newborn Child & Adolescent Health (MNCAH), Expanded Program on Immunization (EPI), emergency health response activities. Supportive supervision, monitoring in all provinces of the country are needed at provincial, regional & national levels.

In addition, in September 2021, the World Bank (WB) suspended the “Sehatmandi” project in Afghanistan. This threatened the collapse of the health system in the country. Our client & WHO have worked to sustain the services through CERF & ARTF - WB fund until a new mechanism is created. A monitoring framework of this modality is being created including monitoring field visits to health facilities receiving support through our client.

Also, in 2021, vaccination against COVID 19 started in the country, adding more burden to the already overwhelmed health system. The introduction of this new vaccine targeting different population groups, required another additional monitoring framework, tools to track the progress, ensue quality & equity.

Purpose & scope of work:

  • Provincial Health Extender is expected to carry out a range of duties, as outlined below, to support monitoring functions, demand generation, accountability to affected population mechanisms, technical facilitation & program implementation. 
  • Those duties, which are described in detail below, do not include neither programmatic nor financial authority. Health Extenders are identified among Afghan citizens with specific expertise & a thorough knowledge of the territory in which they operate, based on their residency & / or work experience.

Role Objectives:

Situation analysis & assessment:

  • Conduct regular situation monitoring in specific areas based on requests by the program or zonal office, to assess the overall humanitarian situation, document the needs of affected populations, in particular women & children health needs.

 

Monitoring:

  • Conduct regular visits to monitor health program activities supported by our client & facilitate their staff programmatic visits.
  • Monitor Implementing Partners (IP's) management of service delivery at our client supported health facilities (under various projects including ARTF /  ADB & others) using the common checklists & tools.
  • Assessing program implementation by IP's engaged through PCAs (Program Cooperation Agreements), SSFAs (Small - Scale Funding Agreements) as well as program implementation by partners & agencies according to our clients Afghanistan country office work plans. 
  • Checking EPI supplies chain systems, equipment in place, their functionality status, including following SOPs in vaccine storage & utilization. 
  • Supporting health information system, data registration reviews, assessments, the implementation of Data Quality Improvement (DQI) plans & microplanning activities. 
  • Verifying health supplies are delivered to target beneficiaries in line with relevant program documents, flag any delays & bottlenecks. 
  • Conducting Post Distribution Monitoring (PDM) of supplies to assess beneficiaries’ feedback in terms of the quality, timeliness & relevance of supplies delivered.
  • Other end user monitoring of specific programs will be conducted as required by the zonal office.
  • Collecting direct feedback from beneficiaries, affected population about the situation, needs, the suitability of the services provided & how to improve.

 

Technical support:

  • Provide technical services to our clients projects primarily in hard to reach areas, through ensuring regular & systematic technical advice, Quality Assurance (QA) & technical guidance to IP's, counterparts, for the efficient implementation of our clients programs, effective achievement of results for the benefit of Afghan children & women.
  • Identify capacity, technical gaps of IP's, counterparts, provide technical support to the to ensure capacity building & on the job training as needed. 
  • Support the implementation of Mobile Health and Nutrition Teams (MHNT) supported by our client through technical support, capacity building, monitor the service implementation, quality & work with partners at the provincial level to improve MHNT program results.

 

Support to implementation:

  • Support the implementation of Community Health Workers (CHWs) activities for management of childhood diarrhea with ORS / zinc copacks, other commodities supported by our client, through technical support, capacity building, monitor the service implementation & the quality of services.
  • Support implementation of community mobilization, community engagement activities & the implementation of Behaviour Change Interventions (BCC) for health related projects.
  • Support EPI program routine immunization activities including but not limited to service delivery, cold chain system, vaccine, non vaccine supplies & information system. 
  • Support emergency preparedness, response activities implementation & monitoring.

 

Support QA:

  • Report any suspected PSEA case directly to the responsible focal point & inform the Program Specialist.
  • Report any suspected fraud case to the Zonal Program Specialist or relevant staff.

 

Coordination:

  • Coordinate with the Regional Health Extender for the monthly work plan / monitoring plan, share the progress & visits repots on timely manner.
  • Other program related activities as suggested by the supervisor.

 

Deliverables:

  • Our client supported emergency activities monitored including HF under the BPHS / EPHS delivery support.
  • Field visits implemented & reported timely with the required quality.
  • Provincial need assessment for implementation of mobile health teams completed.
  • Monthly monitoring of mobile health & nutrition teams service delivery points conducted. 
  • Monthly report of availability of ORS / zinc stock with CHWs submitted. 
  • Coordination & monitoring of community mobilization / engagement activities facilitated.
  • Need assessment of CHWs training for target provinces completed.
  • Training of CHWs supported by our client is monitored.
  • Routine EPI monitoring checklists completed & reported (this checklist includes all components of routine EPI).
  • Oriented on new RI activities e.g. COVID-19 vaccination, accelerated immunization, measles interventions, awareness raising activities (IEC, media broadcast, etc.).
  • Percentage of HFs equipped with SDDs reported.
  • Emergency need assessment of emergency affected areas completed.
  • Situation monitoring reports & needs assessment data is provided.
  • Program monitoring reports are developed, shared with Zonal Office Program Officers / Specialists & Regional Health Extender including progress information, gaps, challenges, supplies monitoring & beneficiaries’ feedback.
  • Partner’s capacity in area of Health Extender technical expertise is built.

Job Requirements

  • Having certificate in health, medicine, public health, nursing, midwife or related field.
  • Minimum of 2 years of experience in in health, medicine, public health, nurse, midwife or related field.
  • Proficiency in data analysis and interpretation.
  • Excellent communication skills for effective collaboration with healthcare professionals and stakeholders.
  • Proven ability to contribute to the enhancement of health programs through data-driven insights.
  • Reporting skills.
